City <-------我住係香港，但先唔先寫得仔細D？例如寫邊區？
First name<----如果我個名叫Chan Tai Mei,那麼個fist name is Tai Mei嗎？那Middle name又是什麼？

First name就是Tai Mei ~

If you use in the first name, MOST westerners (especially COMPUTERS) would take it as First and Middle name. Some people write it like "TaiMei" (no space) to avoid the problem.
In care of: [Parents/Guardians] (may need to write the name)
